Role Summary 

The Registered Nurse (RN) is a member of a collaborative team who is responsible for coordinating the delivery of safe, quality nursing care while working in a community or home-based setting. The fundamental role of the RN is to promote the achievement and maintenance of each patient's optimal level of health and function through assessment, planning, implementation, evaluation, and development of individualized care plans. The RN promotes wellness and self-care with clients and their families/partners in care in their homes and communities. They establish and maintain formal relationships with partners within the community to ensure the continuity of care. The RN develops therapeutic relationships with patients and their families/partners in care and practice within their scope of professional, legal, and ethical standards. This position provides screening, assessment, prevention, treatment, rehabilitation, and support to clients. 

Clinical Practice 

  • Actively engaged in providing direct care to clients within a multi-disciplinary team 
  • Assess the needs of clients, client groups and the community followed by the development, implementation and evaluation of health promotion, illness prevention and client education sessions and programs, adapting to their needs, lifestyle, culture, and socio-economic situations 
  • Assess, plan, initiate, and evaluate appropriate interventions for client; supporting clients and their families to take responsibility for management of their own health needs 
  • Responsible for administering medications, monitoring vital signs, developing and implementing care plans 
  • Design and develop action plans to address client’s needs in collaboration with the client and family and other health care professionals 
  • Maintain competence in clinical skills, managing client care and recognizing recurring themes relevant to the area of practice 
  • Use comprehensive assessment (using objective, subjective, verbal and nonverbal data), critical inquiry, technology, and research to provide safe, competent nursing services relevant to the area of practice. 
  • Evaluate a client’s health status, health concerns, response to illness and ability to manage health care needs and functions as a client advocate 
  • Monitor, observe, and record information relating to the client's physical and psychological status to facilitate evaluation and modification of client care and treatment 
  • Assess learning needs and provides instruction to clients and their families in the areas of health and rehabilitative practices, life-style adaptation and self-care measures 
  • In collaboration with the client/family determine the plan of care for home and links to community agencies; assist client to navigate health system processes and provide the client with strategies to work within the system 
  • Share relevant information with clients regarding health and refers the client to other sources for information when appropriate 
  • Take a lead role within the health care team, which supports a multi-disciplinary approach to care 
  • Coordinate and lead nursing and auxiliary staff activities related to the care and treatment of assigned clients 
  • Provides education to population or client groups 

Leadership 

  • Use current literature/research and performance indicators to support evidence-based practice 
  • Adhere to routine precautions and infection prevention and control techniques according to policy 
  • Proactively seek professional development opportunities for themselves and others 
  • Attend and/or participate in programs to reinforce nursing education and to remain current with developments and changing trends in the nursing field 
  • Act as a mentor and/or preceptor to students, beginning practitioners and practicing nurses by demonstrating care and treatment techniques, explaining routines, and sharing specialized nursing experiences 
  • Maintain a psychologically and physically safe environment and reports all safety hazards 
  • Identify clinical practice issues affecting patient safety quality outcomes and implementing best practice 
  • Document client care activities consistent with policy, system requirements and scope of employment 
  • Follow and adhere to all policy, ethical and legal guidelines 

Statement of Qualifications: 

  • Baccalaureate Degree in Nursing or Psychiatric Nursing, or a Diploma in Nursing with extensive experience in clinical area may be considered 
  • Some nursing experience (1-3 yrs) with a combination of hospital and/or community setting 
  • Obtain and maintain education credentials and certifications specific to practice area 
  • Active or eligible for registration with the College of Registered Nurses and Midwives of Prince Edward Island

External candidates may be eligible to receive a $8,000 Nursing Incentive, plus a $5,000 Rural or $8,000 Student Loan Incentive depending on location of employment. To be eligible for recruitment incentives candidates must hold a 1.0 FTE, this can be a full-time role or a combination of part-time positions. Relocation Assistance may be available to part or full-time candidates.
